Registration Information:

Moving to Provo?

The following information is needed to register a new student:

• Birth certificate
• Immunization record
• Check out form from previous school
• Documentation showing proof of where you live in Provo School District (utility bill, rental agreement, mortgage statement, etc.)

Upcoming Registration Dates:

March 26, 2008 Current 7th graders will receive 8th grade registration materials during wolf pack

March 31, 2008 Registration materials handed out in 6th grade classes

April 4, 2008 6th grade orientation day at Centennial -Registration materials due for incoming 7th grader

New student registration for 2008-2009 school year:  beginning August 6, 2008

First day of school: August 18, 2008
