New Student Registration

New move in students register AUGUST 5-6, 2009 from 9:00 - 2:00 at Centennial.

Moving to Provo?

The following information is needed to register a new student:

• Birth certificate
• Immunization record
• Check out form from previous school
• Documentation showing proof of where you live in Provo School District (utility bill, rental agreement, mortgage statement, etc.)
